Fall 2014 Registration Information
Please make sure that you have all of the following information turned into CCHS prior to the first day of Fall Sport Tryouts/Practice:
- Updated Physical (within 13 months)
- Completed online Concussion Education Course
- Registration Form (with physical clearance confirmation from School Nurse or Athletic Department)
- Completed SportsWare online account

|2014-15 Free Online Concussion Education Course|
The required online concussion education course is now available on the NFHS Learn website! In order to register and participate in high school athletics, all student athletes and their parents must watch the free, online concussion education course through the National Federation of State High School Associations.
This course is composed of 4 units and followed by a brief evaluation. At the conclusion of the evaluation, a course completion certificate will be available through the NFHS account that was set up to view the course.
There is a completion acknowledgement section on the CCHS Athletic Registration form. This course must be completed by both the student athlete and parent/guardian prior to registering for any sport. This course only needs to be completed once per school year.
