Transcript Request

Graduates should complete the form below to request transcripts. Current students should complete the in-school request process as indicated by Counseling.

Please note: this is a transcript request for Central Catholic High School in Lawrence, Massachusetts.

• We are also providing transcripts for graduates of Presentation of Mary Academy (Methuen). Please indicate via the relevant question on the request form that you are a graduate of PMA.
• For transcripts from St. Mary High School (Lawrence), please contact the Sisters of Notre Dame de Namur at 30 Jeffreys Neck Rd, Ipswich, MA 01938 or by phone 978-356-2159.

Notes on Transcripts

• All transcript requests require your electronic signature. This is an official request and all information contained in this request should be considered private.
• Requests are processed within 48 hours Monday-Friday during normal school hours. Requests are not processed over the weekend or during school holidays or vacations.
• During summer vacation, from July through August, requests will be processed one day a week.
• Graduates must provide their email address when ordering official e-transcripts.
• They must also scan and attach a valid photo id as part of the form.
